    heart image The Center for Cardiovascular Disease Prevention at Mount Sinai West welcomes patients for either a one-time consultation or for ongoing counseling and treatment. We take referrals from other health care providers, but you may also make your own appointment to see us.

    A Treatment Plan Created Just for You

    After each visit to Mount Sinai West, you will receive a written summary and treatment plan. If you are already receiving medical care, we will coordinate plans with your primary provider or specialist.
